Welcome to Eric Crawford Family Dentistry! Please be sure and bring your Driver's License and Insurance Card to your visit, as we will need to make a copy for our files.

If you prefer to fill out new patient forms in our office, please come 15 minutes early to ensure enough time to complete them before your scheduled appointment. We accept most insurance as well as Visa, Mastercard, Discover, American Express and Care Credit.
